DECLARATION OF AGREEMENT
Heart to Heart CPR Training

By booking and/or participating in any class or service, I acknowledge that I have read, understand, and agree to the following terms and conditions:

1. A non-refundable deposit of $25 is required to secure my spot in any class.

2. The remaining balance is due prior to or at the start of the scheduled class.

3. I understand that a minimum of 24 hours’ notice is required to reschedule. Failure to provide proper notice, including no-shows, will result in forfeiture of all payments made.

4. I acknowledge that no certification will be issued until full payment is received.

5. I understand that all payments are non-refundable once services have been rendered.

6. I acknowledge that participation in CPR training involves physical activity, and I assume all responsibility and risk associated with participation.

By proceeding with registration, I voluntarily agree to and accept all terms outlined above.
